Damp-Heat Disease in Shaoyang: Characteristics, Main Medications, and Treatment Methods

1) Damp-heat primarily involves the liver, gallbladder, and spleen-stomach.

2) Above Shaoyang, fire is treated; above Yangming, dryness is treated.

3) Shaoyang Sanjiao:

Fluid pathway: dampness;

Qi pathway: original qi, the vital energy; dampness transforms into heat, requiring Shaoyang fire to resolve;

Grain pathway: the reason for liver wood overcoming earth.

4) Damp-heat disease occurs in the Shaoyang Sanjiao.

① Symptoms: bitter mouth, dry throat, dizziness, wiry pulse.

② Medications:

Yin Chen (Artemisia capillaris): Yin Chen Hao Tang (Yin Chen Decoction);

Zhi Zi (Gardenia fruit): Zhi Zi Chi Tang (Heat); Zhi Zi Bai Pi Tang, Yin Chen Hao Tang (Damp-heat); Huang Lian Jie Du Tang (Huang Lian Detoxification Decoction), Zhi Zi Jin Hua Tang (Heat toxin, Si Huang plus Zhi Zi, can also add Gan Cao for anti-inflammatory and detoxifying effects).

Gan Cao (Licorice): anti-inflammatory, detoxifying, anti-shock; moderates the bitter coldness of Huang Lian, Huang Qin, etc.; softens the harsh purging of Da Huang.

Chi Shao (Red Peony): 30-50g, treats stasis-heat internally; adding Chi Shao 30-60g to Yin Chen Hao Tang for jaundice enhances effectiveness.

③ Da Chai Hu Tang & Xiao Chai Hu Tang: 1. Add Shao Yao: liver purging method; 2. Remove Ren and add Zhi Shi, Da Huang: if solid, then Yangming.

Yin Chen Hao plus Shao Yao Tang: Yin Chen, Zhi Zi, Da Huang, Shao Yao

Da Chai Hu Tang: Chai Hu, Huang Qin, Da Huang, Shao Yao, Ban Xia, Sheng Jiang (for vomiting), Zhi Shi (for secret), Da Zao

Chai Hu soothes the liver, Yin Chen resolves jaundice; Huang Qin benefits the gallbladder, Zhi Zi has anti-inflammatory properties.

5) Liver purging method.

Wu Suan Huan Gan Tang

Chai Hu 12, Chi Bai Shao each 30, Zhi Shi 15, Zhi Gan Cao 6, Vinegar Xiang Fu 9, Vinegar Yan Hu 15, Jiang Huang 9, Sheng Shan Zha 30, Chuan Lian Zi 6, Qing Mu Xiang 6

For stones: add Ji Nei Jin 30, Jin Qian Cao 30, Hai Jin Sha 15

Due to pain from the disease, transitioning from the qi aspect to the blood aspect, for rapid effect, do not use warming tonics like Dang Gui, Chuan Xiong, but use Sheng Shan Zha: purging the liver, blood aspect, and relieving pain.

Main treatment:Gallbladder diseases. Originating from Da Chai Hu Tang, the liver purging method is used to the extreme;

Xie Qing Wan (Liver Purging Pill)

Xie Qing Wan: Long Dan, Qing Dai, Zhi Zi, Da Huang (wine-fried), Chuan Xiong, Dang Gui, Fang Feng, Qiang Huo

Modified Xie Qing Wan: Long Dan 6, Da Qing Ye 15, Zhi Zi 6, wine Da Huang 6, Chuan Xiong 6, Dang Gui 12, Fang Feng 6, Qiang Huo 6, Sheng Gan Cao 6.

Gan Cao: anti-inflammatory, detoxifying, antiviral, alleviates abdominal pain and diarrhea caused by Da Qing Ye, Qing Dai, Da Huang, etc.

Clear the liver: Long Dan Cao, Qing Dai, Zhi Zi;

Nourish the liver: Dang Gui, Chuan Xiong (Ben Tun Tang);

Raise Yang and disperse fire: Qiang Huo, Fang Feng;

Xie Qing Wan is more heat-heavy compared to Long Dan Xie Gan Tang.

Damp-Heat Three Non-Contradictions: Shao Yao, Chai Hu, Sheng Di.

Shao Yao: diuretic, removes dampness, as in Zhen Wu Tang, Da Yuan Yin, Yin Chen Hao Tang with Shao Yao.

Chai Hu: Chai Hu intercepts liver yin, generally contraindicated in warm diseases, needs to be combined, such as Zi Shui Qing Gan Yin; after transforming heat and injuring yin, can replace Chai Hu with Qing Hao.

Sheng Di: Xue Sheng Bai says that nourishing it will not resolve the deep disease; damp-heat disease requires nourishing yin with Sheng Di, needs to be combined with Lu Gen, Shao Yao, Ze Xie, etc.

Mechanism and Symptoms of Heavy Dampness and Heat in Jaundice.

Damp-heat: occurs in Taiyin, the person mustvomit;

Heavy heat: occurs in Yangming, the person shivers andhas fever.

Diagnosis of jaundice, besides tongue and pulse, mainly looks at vomiting, fever or not

Prognosis of jaundice: thirst with jaundice is difficult to treat, while no thirst with jaundice is treatable. For example, liver cirrhosis with jaundice, ascites.

Patterns of Damp-Heat in the Liver and Gallbladder

1) Liver and gallbladder damp-heat:Gan Lu Xiao Du Dan. Subsequently, there are twenty-four methods for Gan Lu Xiao Du Dan.

2) Stomach fluid affected, gallbladder fire surging pattern (Shaoyang Yin deficiency):

15 damp-heat pattern, four to five days, great thirst, chest tightness, dry retching, thin rapid pulse, tongue bright like a mirror,

should use watermelon juice, golden juice, fresh Sheng Di juice, sugarcane juice, take Yu Jin, Mu Xiang, Xiang Fu, Wu Yao, etc.;

This situation is often seen after chemotherapy, also with nausea and vomiting. Teacher Wu handles this situation, does not use 15 methods, often uses orYi Guan Jian plus Yin Chen, Nuo Dao Gen (rice sprouts), Lu Gen, Pi Pa Ye, Zhu Ru etc.

3) Wood fire surging upwards (Shaoyang with dampness):

16 damp-heat pattern,vomiting clear water or phlegm, damp-heat retained internally, wood fire surging upwards. Should useWen Dan Tang plus Gua Lou, Bi Yu San, etc.;

Wu Xiong Zhi: commonly usesGan Lu Xiao Du Dan for modification.

4) External heat invading, internal heat closing:

Use Chao Pu, Yu Jin, such as Di Tan Tang, Huang Lian Wen Dan Tang.

5) Gallbladder qi not smooth pattern:

27 damp-heat pattern, treat according to method, all symptoms retreat, but at nightpalpitations and nightmares, remaining evil retained, gallbladder qi not smooth, should use wine-soaked Yu Li Ren, ginger-fried Jujube seed, pig gallbladder skin, etc.

Wu: can useHe Huan Tang. Huang Lian 6, Ban Xia 30, Xia Ku Cao 30, Rou Gui 1, Fu Ling 50, Yu Jin 30, He Huan Pi 30, Zhi Gan Cao 3, Chao Pu 9, Yuan Zhi 30, Zhu Ru 30, Zhi Shi 9, Jujube seed 30, Dan Xing 6-15, Sheng Jiang 30, Yu Li Ren 15

Disorder of Lung and Stomach Harmony

Severe vomiting, cannot take soup, first useSu Ye Huang Lian Tang in light doses frequently, to open the stomach to take medicine.

17 damp-heat pattern, vomiting and nausea persist, day and night not healing, wanting to die, lung and stomach not harmonized, stomach heat moving to the lung, the lung does not receive evil. Should use Chuan Lian 3-4 fen, Su Ye 2-3 fen, decoct these two herbs, and it will stop immediately.

Su Ye:stops vomiting and candetoxify, as in Su Chan Di Huang Tang, An Shen Yin for treating acute renal failure.

An Shen Yin:Su Ye 9g, Huang Lian 6g, Ban Xia 9g, Fu Ling 9g, Sheng Jiang 6g, Gan Cao 3g, Zhi Shi 6g, Lu Gen 30g, Zhu Ru 15g, Tu Fu Ling 30g, Bai Mao Gen 30g. Main treatment:renal failure, kidney damage, reduces urea nitrogen, creatinine, effective for chemotherapy-induced kidney damage.

Modification: for constipation add Da Huang.

Comparison with Gan Jiang Huang Qin Huang Lian Ren Shen Tang: Cold: Huang Lian/Huang Qin; Warm: Su Ye/Gan Jiang, Ren Shen. Wu personally feels that for truly severe vomiting, if soup cannot be taken, Su Ye Huang Lian Tang taken frequently is more direct, small doses of Su Ye Huang Lian Tang are actually more effective.

Dosage: Su Ye 3-6g, Huang Lian 3g.Decoct once, boil for 5 minutes, no need to finish the medicine, can start with one sip, later can drink two sips after a while, no need to finish the medicine.

Damp-Heat Disturbing Up and Down

Disturbing upwards:Pi Pa Qing Gan Yin (Pi Pa Ye);

Damp-heat descending:Chai Miao Yin (Niu Xi).

Damp-Heat Disease Three Jiao Medication Patterns

Damp-Heat Disease Three Jiao Medication Patterns: Lung and stomach treated together.

1) Xing Ren, Kao Ren, Yi Ren (San Ren Tang);

2) Xing Ren, Hua Shi, Tong Cao ( Xing Ren Hua Shi Tang);

Heat and humidity, all three jiao affected, tongue gray-white, chest tightness, tidal fever, vomiting, thirst with self-benefit, sweating and short urination, Xing Ren Hua Shi Tang is the main treatment.

Use Xing Ren (3 qian) Hua Shi (3 qian) + Huang Qin (2 qian) Huang Lian (1 qian) Ban Xia (3 qian) + Cong Po (2 qian) Chen Pi (1.5 qian) Yu Jin (2 qian) Tong Cao (1 qian).

Yu Jin is essential for damp-heat with swollen tongue edges.

3) Pi Pa Ye, Zhu Ru, Lu Gen (Xue Shi Wu Ye Lu Gen Tang).

“Yi Yuan”: “The treatment method mainly focuses on lightly opening lung qi, as the lung governs qi, and when qi transforms, dampness transforms itself. Even if there is a combined evil, it will also transform together.Damp-heat treats the lung, a timeless conclusion.Jie Geng, Zhi Ke, Su Geng, Bo He”.

Damp-Heat Entering the Lung Collaterals

Two types of cough: summer cough (Liu Yi San plus Pi Pa Ye, Ting Li Zi),

chest cavity adhesion cough

Yu Ke Tang: Xiang Fu 9, Xuan Fu Hua 9, Yi Yi Ren 30 + Tao Ren 9 Dang Gui 9, Wu Gong 3 San Qi 6, Bai Jie Zi 9, if there is pleural effusion add Ban Zhi Lian 30, Ting Li Zi 30, for pain add Jiang Xiang 3.

Derived from Xiang Fu Xuan Fu Hua Tang, damp-warmth with rib pain or cough, or not coughing, no cold, but tidal heat or even cold-heat like malaria, do not misinterpret as Chai Hu pattern, Xiang Fu Xuan Fu Hua Tang is the main treatment: if not resolved for a long time, intermittently use Kong Yan Dan. (Bitter, spicy, and light combined with aromatic opening collaterals method) Sheng Xiang Fu (3 qian) Xuan Fu Hua (3 qian) Su Zi (3 qian) Guang Pi (2 qian) Ban Xia (5 qian) Fu Ling (3 qian) Yi Ren (5 qian) water eight cups.

This chest cavity mucosal adhesion often does not respond well to conventional methods, this formula is effective, pleural effusion in non-acute phase does not need to use Gan Sui and other purgatives.

Shaoyang with Dampness and Its Formulas

1) Da Yuan Yin: the pathogenesis is in Shaoyang, the evil is thick white like accumulated powder for Yangming; derived from Chai Hu Da Yuan Yin.

2) Gan Lu Xiao Du Dan: middle jiao (also used for external pathogens), (Chai Miao Yin for lower jiao, Hou’s black powder for gallbladder fire in upper jiao).

3) Long Dan Xie Gan Tang: internal injury.

4) Hao Qin Qing Dan Tang: transforms out of Shaoyang (also used for hidden pathogens).

For hidden summer heat (equivalent to Western medicine’styphoid fever), the initial stage of typhoid fever is Hao Qin Qing Dan Tang pattern, the core medicinal herbs areQing Hao, Huang Qin, Da Qing Ye, and Qing Dai, directly targetingtyphoid bacillus. Wu Xiong Zhi does not use Qing Dai, generally Da Qing Ye 30-50g plus Gan Cao, Qing Dai is prone to gastrointestinal discomfort after taking.

Hidden Pathogen Warm Disease Ancestor Formula:Huang Qin Tang

Hidden pathogens transform out of Shaoyang, as in Liu Bao Yi’s “Warm Heat Encountering Source”, using Huang Qin Tang plus Dou Chi, Xuan Shen. Also, Da Yuan Yin, this hidden pathogen warm disease with dampness.

Modification formulaSheng Yang Bai Du San: viral infections recurring, belong to hidden pathogens.

Sheng Yang Bai Du San:Sheng Ma 30, Huang Qin 9, Dan Pi 9, (Da Qing Ye 30, Gan Cao 6), Yi Yi Ren 90, Dan Zhu Ye 30, Bai Dou Kou 6, Jiang Huang 30, Wu Gong 3.

Main treatment: viral infections, recurring, this belongs to hidden pathogens.
