The ancient method of diagnosis in Chinese medicine does not merely adhere to the modern practices of observation, listening, inquiry, and palpation, known as the four examinations. In the practice of ancient Chinese medicine, observation (望诊, wàng zhěn) holds a very high status, concealing within it the mysterious and profound realm of Tianmu (天目, tiānmù).

The highest realm of diagnosis in ancient Chinese medicine is observation, which is not limited to the superficial phenomena such as complexion and tongue coating as described in modern Chinese medicine, but delves into the deeper mysteries of the human body. Renowned ancient physicians like Bian Que (扁鹊, Biǎn Què) and Cang Gong (仓公, Cāng Gōng) opened their Tianmu through specific herbal formulas, allowing them to perceive the spatial location of diseases and directly observe the low spiritual evil qi causing the ailments.
The Tianmu essentially refers to the pineal gland located in the center of the brain. Modern research has discovered that the pineal gland possesses structures similar to the eyes and contains numerous photoreceptor cells and magnetic crystals, with functions that far exceed what our naked eyes can see. Through the Tianmu, ancient Chinese medicine practitioners could gain insight into the deep structural aspects of the body, directly observing different facets of the human body, and even seeing the spiritual aspects referred to in Chinese medicine.
Although modern medicine can observe different aspects of the human body to a certain extent using MRI, CT, and PET imaging technologies, the realm of Tianmu in ancient Chinese medicine offers a more direct and profound method of observation. It is not limited by physical devices and can directly perceive the essence of the human body and the sources of diseases.
However, the realm of Tianmu is not accessible to everyone. It requires the practitioner to possess profound medical ethics and insight, and to open it through specific methods and formulas. This is precisely the depth of the ancient Chinese medicine observation method; it is not merely a medical skill but a profound understanding and insight into life and the universe.
