The current state of confusion, feeling like collapsing when getting up, and a weak pulse clearly indicate the decline of true Qi, unable to support. It is advisable to strongly tonify Yang, thus using Si Ni Decoction to restore Yang.9、Question:Why does a patient have a continuous runny nose with clear mucus, sneezing incessantly, and after taking all kinds of external pathogen dispersing medicines, it worsens?Answer: This is due to insufficient pre-natal true Yang, unable to control the fluids. The external pathogen’s clear mucus and sneezing must present with fever, headache, body pain, fear of cold, and nasal congestion. It is advisable to greatly tonify pre-natal Yang. If it is determined to be excess fire, or a mix of cold and heat, it is all misdiagnosis. According to discussions with Zheng’s direct descendant and my own experience, his grandfather had a son late in life, and his father was weak and sickly, so the family always kept Fuzi Li Zhong Wan and took it, and his health gradually improved. I also often use this formula to treat patients with Spleen and Kidney Yang deficiency, who are weak and have recurrent colds, achieving satisfactory results.

The formula should be Qian Yang Dan.【Explanation】 In clinical practice, if there is foul odor without Yin symptoms, it is often due to excessive stomach fire, treated with Bai Hu Decoction plus Ren Shen. If there is excessive Yin forcing Yang outward and causing foul odor, a large dose of Fuzi Li Zhong Decoction with modifications can be used.13、Question: Why does a patient suddenly lose the ability to move the tongue, limbs become unable to lift, and during sleep, saliva flows out unconsciously?Answer: This is due to excessive Yin and the inability of Yuan Yang to stabilize and operate. The entire body’s joints and orifices rely on true Qi for protection and movement. This condition can be treated with Fuzi Li Zhong Decoction with added Sha and Ban.When Yang is deficient, the strength of the middle palace is weak, and the dampness from food and fluids, along with internal Yin Qi, accumulates, leading to phlegm and saliva. Over time, if Yang is weak, cold phlegm rises, blocking the clear orifices, leading to sudden collapse and confusion, referred to as phlegm obstructing the middle. It is advisable to revive Yang, descend rebellious Qi, and warm the middle to gather Qi back to the root. The formulas should be Si Ni Decoction, Feng Sui Dan, or Qian Yang Dan.16、Question: Why does the chest and abdomen hurt severely, with a face red as vermilion, no desire for tea or water, and a need for heavy objects to press down to feel slightly better, otherwise feeling like dying?Answer: This indicates that Yuan Qi is violently escaping and contending with Yin. This condition cannot be treated without gathering Qi back to the root and reviving Yang, thus using modified Fuzi Li Zhong Decoction or Qian Yang Dan.【Explanation】 Chest and abdominal pain can originate from nine types, namely: “1. As for Gui Ling Zhu Gan Decoction, it is a key formula for treating phlegm, water, and dampness.18、Question: Why does a patient suddenly have both breasts swell like plates, with normal skin color, slight pain, feeling heavy and preferring to lie down, with no desire for any food?Answer: This indicates that excessive Yin is causing the Yuan Qi to rise in the liver and stomach. This condition should be treated with Fuzi Li Zhong Decoction plus Wu Zhu Yu (Evodia). If the nipples are not swollen, the disease is focused on the stomach; if the nipples are swollen, the disease is focused on the liver. Understanding the release of one Yuan, the treatment methods will not deviate from reviving Yang, gathering Qi, sealing marrow, and subduing Yang. If it is thought to be caused by wind-cold or Qi stagnation, there must be alternating cold and heat, headache, body pain, severe redness and swelling, thirst, and various symptoms, then it can be treated with Qi-regulating, blood-activating, and dispersing formulas.19、Question: Why do both sides of the abdomen suddenly swell, appearing red like vermilion, with slight pain, feeling heavy, and bluish-black nails?Answer: This indicates that excessive cold in the Jueyin is forcing out the Yuan Yang. It is advisable to revive Yang and dispel Yin, thus using Si Ni Decoction with added Wu Zhu Yu. If it is liver channel wind-fire, there will be severe pain, and the body will not feel heavy, and the nails will not be bluish-black. Now, only the signs of excessive cold in the Jueyin are seen, thus it is known that the Yuan Yang is being forced out by cold.20、Question: Why does a patient experience severe emaciation in the head, face, and limbs, with a lower abdomen as large as a gourd, bluish lips, no desire for food, and shortness of breath?Answer: This indicates that Yang deficiency is obscured by Yin. It is advisable to strongly tonify Yang, thus using Si Ni Decoction or Fuzi Li Zhong Decoction with added Sha and Ban.What are the external causes? It may be due to wind, cold, heat, dampness, dryness, or heat entering the interior, blocking the Qi mechanism, prolonged over time, leading to stagnation of essence and blood. If the invasion is shallow, it flows to the skin; if deep, it flows to the abdomen. If it is in the joints of the hands and feet, it leads to sores, ulcers, and toxic boils. If it is blocked in the upper burner, it leads to chest obstruction; if in the middle burner, it leads to fullness; if in the lower burner, it leads to abdominal fullness.What are the internal causes? Generally, those entering from the outside cause Qi blockage, while those exiting from the inside cause Qi dispersion. For blockage, it is advisable to open, regulate Qi, and move blood; for dispersion, it is advisable to gather, revive Yang, and warm and tonify first. However, there is a difference between distension and swelling; distension is from Qi, and pressing it feels solid but empty inside; swelling is from blood, and pressing it feels solid inside and out. To treat distension, it is advisable to nourish Qi, tonify Qi, and gather Qi, avoiding breaking Qi, exhausting Qi, and moving Qi, especially emphasizing nourishing blood. To treat swelling, it is advisable to activate blood, move blood, and break blood, avoiding cooling blood, stopping blood, and restraining blood, especially needing to move Qi. If learners want to understand the key to treating distension, it lies in discerning the Qi aspect to determine deficiency or excess.21、Question: Why are the bowel movements not smooth, and for three to five days there is no feeling of fullness, with abdominal pain, a bluish tongue, and no desire for food?Answer: This indicates that the Yang in the lower burner is deficient, unable to transform the Yin in the lower burner. It is advisable to warm and tonify the Yang in the lower burner, thus using Si Ni Decoction with added An Gui. If there is heat accumulation causing difficulty in bowel movements, the person will be unusually irritable, and there will be yellow or white tongue coating, preferring cold water, and foul breath. For difficulty in bowel movements, it is advisable to nourish blood, clear heat, and moisten dryness, using Ma Ren Wan; for difficulty in urination, it is advisable to nourish Yin and clear heat, using Dao Chi San.

It is advisable to support Yang and suppress Yin, thus using Fuzi Li Zhong Decoction with added Sha and Ban.23、Question: Why does a healthy person feel no illness, but after urination, there is a continuous drop of semen like threads, and in severe cases, it does not stop?Answer: This indicates that the pre-natal Yang is declining, unable to restrain the essence. It is advisable to greatly tonify Yuan Yang, focusing on the interaction between heart and kidneys. The formula should be Bai Tong Decoction.24、Question: Why does a patient experience swelling in both legs up to the knees, feeling as cold as ice?Answer: This indicates that the Yuan Yang in the lower burner has not been stored, and the Yin Qi has not been restrained. It is advisable to strongly tonify Yuan Yang, facilitating the interaction between upper and lower, allowing water and fire to mutually support, thus the swelling will naturally subside. The formula should be Bai Tong Decoction.25、Question: In Shaoyin disease, with vomiting and diarrhea, cold hands and feet, and restlessness, why is Wu Zhu Yu Decoction indicated?Answer: Vomiting leads to the loss of Stomach Yang, and diarrhea leads to the loss of Spleen Yin, resulting in the loss of both Yin and Yang in the middle palace, causing Yang Qi to fail to reach the extremities, hence the coldness. The two Earths reside in the middle, one transporting essence upwards to the heart and the other transporting essence downwards to the kidneys. Now, due to excessive vomiting and diarrhea, both Earths are suddenly deficient, unable to transport essence and facilitate interaction between upper and lower, hence the restlessness and desire to die. Wu Zhu Yu Decoction is indicated.
