🔸Insufficient Middle Yang:Spleen and stomach Yang deficiency actually refers to insufficient Yang Qi in the middle jiao! When Yang Qi is insufficient, warmth is lost, and one will first exhibitcold limbs.
The stomach governs the rotting and ripening of food, while the spleen governs transportation and transformation. When warmth and movement are deficient, one often feels bloated in the stomach, lacking appetite, and unwilling to eat; coldness causes stagnation and contraction, leading to cold obstructing the stomach channels, resulting in pain, often feelingabdominal pain, especially after exposure to cold.
The spleen Qi governs upward movement, while the stomach Qi governs downward movement. When the upward movement is deficient, consuming cold foods will cause discomfort, easilydiarrhea, with frequent loose stools; when the downward movement is deficient, stomach Qi rebels, leading tovomiting.
With these symptoms, one can basically determine it as spleen and stomach Yang deficiency, and the treatment principle is primarily to warm the middle and disperse cold, with supplementary tonification of Qi and strengthening the spleen, suitable forLi Zhong Wan (Regulate the Middle Pill).

This formula is simple, consisting of only four herbs:Gan Jiang (Dried Ginger) 6g, Ren Shen (Ginseng) 6g, Bai Zhu (White Atractylodes) 6g, Zhi Gan Cao (Honey-Fried Licorice) 6g.
🔸Gan Jiang is very pungent and hot, entering the middle jiao (spleen and stomach), warming the middle and dispersing cold, supporting the Yang Qi of the middle jiao, and suppressing the cold evil of the middle jiao, serving as the monarch herb.
🔸Ren Shen serves as the minister herb, tonifying Qi and strengthening the spleen, replenishing Qi to restore Yang.
🔸Bai Zhu serves as the assistant herb, drying dampness and aiding the spleen’s transportation, as the spleen prefers dryness and dislikes dampness; by drying dampness, it helps the spleen and stomach’s function, working with Ren Shen to strengthen spleen Qi and restore normal function.
🔸Zhi Gan Cao, assists Ren Shen and Bai Zhu in strengthening the spleen, with a sweet flavor that alleviates pain, particularly for abdominal pain; it harmonizes the other herbs, serving as the envoy herb.
The combination of these four herbs warms the middle Yang, tonifies spleen Qi, and aids in transformation, making it the “exclusive medicine” for spleen and stomach Yang deficiency. The above four herbs can be prepared as a decoction, or one can directly choose the Chinese patent medicineLi Zhong Wan (Regulate the Middle Pill).
