First, let us understand these two forms of TCM preparations. Pills (wanji) and decoctions (tangji) are two important forms of traditional Chinese medicine. Pills refer to spherical or quasi-spherical preparations made from finely powdered herbs or herbal extracts combined with suitable binders or other excipients. The main characteristics are: 1, slow dissolution and drug release, which can prolong efficacy, reduce toxicity and irritation, and minimize adverse reactions; suitable for chronic disease treatment; 2 , pills made with water-soluble matrices, dissolve quickly, and act rapidly.Decoctions refer to liquid preparations made by boiling or soaking herbs to extract their juices. Decoctions are the earliest and most widely used form of TCM in our country. The main characteristics are: 1 , they can adapt to the needs of TCM differential diagnosis, where the composition and dosage of the prescription can be adjusted flexibly according to changes in the patient’s condition; 2 , decoctions are liquid preparations that are absorbed quickly and can exert their effects rapidly; 3 , they are simple and easy to prepare.
In summary, the characteristics of pills and decoctions show that pills have the characteristic of “long-lasting efficacy,” while decoctions have the characteristic of “rapid efficacy.” However, in clinical practice, the choice between pills and decoctions cannot be made solely based on these characteristics. For example, some patients may not tolerate the taste of decoctions, some may have poor gastrointestinal absorption of decoctions, and some may need to take medication long-term without the means to prepare decoctions. In these cases, pills can be chosen for administration.
