Wang Zhen (Observation Diagnosis) is a part of the essence of TCM theory developed over thousands of years, and is one of the four diagnostic methods in TCM: “Wang, Wen, Wen, Qie”. A healthy person’s face should be clean, with a rosy and shiny complexion.
Newborns typically have clean faces; however, as they grow older and are exposed to various factors such as air, water, food additives, environmental pollution, genetic changes, and mental stress, their health may be compromised, leading to symptoms appearing on the face.
Facial Color:
Yellow: Indicates issues with the spleen and stomach, and the digestive system;
White: Indicates issues with the lungs and respiratory system;
Red: Indicates issues with the cardiovascular system;
Green: Indicates issues with the liver and gallbladder, and the immune system;
Black: Indicates issues with the kidneys and bladder.
Facial Features:
The face of an infant is clean, but as they transition to solid food after weaning, the abilities of the heart, liver, spleen, stomach, and kidneys become apparent. As one ages, various conditions manifest on the face, such as acne, blemishes, scars, and wrinkles. Generally, adolescents show acne, middle-aged individuals show wrinkles, and the elderly show blemishes.
Birthmarks and Moles on the Face: Indicate congenital functional deficiencies of the corresponding organs;
Spots on the Face: Indicate chronic diseases formed by long-term chronic consumption (3-5 years);
Acne on the Face: Indicates current inflammatory changes in the corresponding organs (short-term formation);
Widespread Acne and Spots: Indicate endocrine disorders or decreased liver immune function.
Prominent Eye Bags—Check the Spleen and Stomach
The relationship between the eyes and the internal organs (five zang and six fu) is particularly close
❖ Eyelids correspond to the spleen
❖ The white part of the eyeball corresponds to the lungs
❖ The black part corresponds to the liver
❖ The pupil corresponds to the kidneys
❖ The blood vessels in the eyeball correspond to the heart
When our eye bags are very prominent, it may indicate spleen and stomach deficiency. You can take 10 grams of Goji berries (Gou Qi Zi), 5 grams of Chrysanthemum (Ju Hua), and a little rock candy (not for diabetics) to brew as a tea.
Ears—Assessing Coronary Heart Disease
Pay attention to whether there is a prominent line between the earlobe and the auricle. If there is, it is essential to strengthen heart care, especially for middle-aged and elderly individuals, as it indicates a higher incidence of coronary heart disease.
❖ If the ears are rosy and shiny, and soft and thick, it indicates abundant qi and blood, and normal organ function;
❖ If the ears are bluish or uneven in color, it indicates cold syndrome in the body and the presence of blood stasis;
❖ If the ears are pale and thin, it indicates qi and blood deficiency, spleen and stomach weakness, and cold syndrome;
❖ If the ears are dry and blackened, it indicates kidney deficiency and possible severe illness.
Nose—Indicating Lung Function
Traditional Chinese Medicine believes that “the lungs open to the nose,” meaning that both the appearance and function of the nose (i.e., breathing and smelling) are closely related to lung function.
❖ A red nose indicates lung heat;
❖ Enlarged pores and oily nose indicate damp-heat in the spleen and stomach;
❖ A pale nose tip indicates blood deficiency or anemia;
❖ A bluish nose indicates liver qi stagnation;
❖ A dry and dull nose indicates a severe condition;
❖ Rhinitis and colds are mostly caused by lung heat or lung cold.
Pale Lips—Risk of Infertility
The lips correspond to the spleen, and the mouth and surrounding area correspond to the stomach, so issues around the mouth and lips should be traced back to spleen and stomach causes.
❖ Toothache, swollen gums, and bad breath are mostly due to stomach fire;
❖ Sweet or sticky mouth and aversion to greasy foods are mostly damp syndrome. If you often lack appetite, it indicates spleen and stomach deficiency; you can eat millet, pumpkin, yam, and hawthorn;
❖ Frequent mouth ulcers indicate excessive heart fire; you can take 10 grams of Chrysanthemum, 15 grams of Honeysuckle (Jin Yin Hua), and 10 grams of Ophiopogon (Mai Dong) brewed as tea;
❖ If the lips are pale, it indicates qi and blood deficiency; you can eat black rice, peanuts, millet, yam, black chicken, and longan;
❖ If the lips are bluish-purple, it indicates liver qi stagnation or heart disease; frequent dry and cracked lips indicate yin deficiency and internal heat, and you can eat pears, lotus root, white fungus, and mung beans.
Tongue—Checking for Heat
When a person’s tongue tip is particularly red, it may indicate that their heart fire is too strong.
Some elderly individuals can tell that a child has overeaten just by looking at the thick and greasy tongue coating and the sour smell in the mouth.
❖ A red or deep red tongue indicates internal heat, or “fire”; it is recommended to clear heat and detoxify;
❖ If the tongue is bluish-purple, it indicates blood stasis, which can also be seen in drug or food poisoning; if it is poisoning, seek medical attention immediately.
❖ If the tongue is pale, it indicates deficiency (such as qi and blood deficiency, anemia, etc.); it is necessary to tonify deficiency and support the righteous qi.
Hand Diagnosis methods are very simple and easy to learn, even those with no medical knowledge can accurately diagnose diseases using the methods described. How to Observe During Hand Diagnosis
“
When a person extends their hands towards you, the first step is observation. A healthy person’s palms should be pinkish-white, moist, and elastic.
If you see a palm that is dull, yellowish, or whitish, it indicates a person with qi and blood deficiency and malnutrition. The texture of the palm flesh is poor, and a dry palm indicates digestive issues. The spleen governs the muscles, so spleen and stomach issues are significant.
If the palm color is significantly red, it indicates a person with yin deficiency and excessive internal heat, and liver fire is also likely to flare up. If the palm is as red as cinnabar, it is even more serious, but you should ask if this is a temporary redness due to recent alcohol consumption or excessive supplements.
If the fingers turn darker than the palms, becoming purple or dark, it indicates heavy internal cold, and blood circulation has slowed down, leading to increased blood viscosity.
The focus of observation is the blue veins, which are visible under the skin. The presence of blue veins indicates that physiological waste such as blood stasis, dampness, heat toxins, and stagnation cannot be expelled from the body, reflecting the accumulation of waste within the body.
If several blue veins are visible on the palm, it indicates long-term constipation. The thicker the blue veins, the more severe the problem. If the blue veins are raised, it is even more serious. If you have several bowel movements a day, the blue veins will recede. After a period of recovery, the blue veins will disappear.
Blue veins on the outer side of the thumb or the joints indicate insufficient oxygen and blood supply to the head, often leading to headaches and dizziness. This is simple to understand; this area represents the junction of the neck and brain, which is the reflection area for the carotid artery and blood supply to the head, so the appearance of blue veins here mainly indicates head issues.
The blue veins on the outer side of the thumb correspond to oxygen deficiency, while the blue veins in the joint seams correspond to blood supply deficiency. If there are blue veins at the base of the thumb, it indicates coronary artery hardening. This is the reflection area for the heart.
The thicker the blue veins, the more severe the problem. If the blue veins are thin and shallow, it indicates a short duration of illness. At this time, the patient will not have obvious discomfort in the heart, but may experience chest tightness or pain when tired or in a bad mood, which will improve after resting.

The right hand's tiger mouth palm surface quickly locates the liver.
Poor vision has coarse lines, fine lines indicate poor tendons,
The palm color is red, blue, or gray, indicating stomach issues.
Messy lines at the base of the thumb indicate early prevention of stomach diseases,
The left hand's tiger mouth palm surface is a good indicator of the spleen.
Regularly massaging in the morning and evening strengthens the body, fresh breath, and strong immunity,
For women, check the right hand for breast health, and check the horizontal lines on the wrist on both sides.
If uneven or lumpy, it indicates early prevention of breast hyperplasia,
The index finger's nail indicates cerebral blood vessels, prominent ridges indicate hardening.
For women with dysmenorrhea and blood clots, the middle finger's base has prominent blue veins,
Reproductive diseases are the hardest to diagnose; remember male left and female right.
