In Traditional Chinese Medicine (TCM), constipation has three meanings:
1. Feces remain in the intestines for too long, causing blockage and prolonged defecation cycles;
2. The cycle may not be long, but the feces are hard and difficult to expel;
3. The feces may not be hard, but there is a sense of urgency without smooth expulsion.
This roughly corresponds to functional constipation, irritable bowel syndrome, constipation due to rectal and anal disorders, and drug-induced constipation in Western medicine. TCM categorizes constipation into four types based on different syndromes: Heat Accumulation in the Stomach and Intestines (Re Jie Chang Wei Xing), Qi Stagnation and Accumulation (Qi Zhi Yu Jie Xing), Fluid Deficiency and Dry Intestines (Jin Kui Chang Zao Xing), and Yang Deficiency and Cold Congealing (Yang Xu Han Ning Xing).
1.Heat Accumulation in the Stomach and Intestines
Symptoms: Hard stools, short and red urination, flushed face and body heat, possibly accompanied by abdominal distension and pain, dry mouth, bitter taste, and bad breath. The tongue is red with a yellow greasy or dry cracked coating.
Treatment method: Clear heat and guide stagnation, moisten the intestines and unblock the bowels.
Medicinal herbs: Qing Ning Wan, Yi Qing Granules, Qing Xie Wan, Tong Bian Ling Capsules, Da Huang Tong Bian Pian, Tong You Run Zao Wan, Zhi Zi Jin Hua Wan, etc.
2.Qi Stagnation and Accumulation
Symptoms: Constipation, inability to defecate, belching, chest and hypochondriac fullness, intestinal rumbling and abdominal pain, poor appetite. The tongue coating is thin and reversed.
Treatment method: Regulate Qi and guide stagnation, descend and unblock the bowels.
Medicinal herbs: Bing Lang Si Xiao Wan, Si Xiao Wan, Mu Xiang Bing Lang Wan, Kuan Zhong Shun Qi Wan, Si Mo Tang Oral Liquid, etc.
3.Fluid Deficiency and Dry Intestines
Symptoms: Constipation, pale complexion, dizziness, palpitations, dry mouth. The tongue is pale with little coating.
Treatment method: Nourish blood, moisten dryness, and unblock the bowels.
Medicinal herbs: Ma Ren Zi Zhi Pi Wan, Ma Ren Wan, Run Chang Capsules, Qing Shu Granules, Tong Le Granules, Run Chang Tong Mi Tea, etc.
4.Yang Deficiency and Cold Congealing
Symptoms: Difficult and painful defecation, clear and long urination, aversion to cold and preference for warmth, pale complexion, cold hands and feet, possibly accompanied by abdominal cold and pain.
Treatment method: Warm the interior, disperse cold, unblock the bowels, and relieve pain.
Medicinal herbs: Cong Rong Tong Bian Oral Liquid, Gui Fu Di Huang Wan combined with Ma Ren Run Chang Wan.
Note on the use of Chinese patent medicines: Constipation in TCM is generally divided into the four types mentioned above, with different cold and heat, deficiency and excess conditions requiring different medications. Treatment should be tailored to the specific situation.
