Tips for Topical Medications in TCM

Applying topical medications may seem straightforward, but there are many misconceptions. It is not necessarily better to apply the medication more frequently or to use a thicker layer of ointment. Sometimes, people rely on their experience to apply topical medications, but the results may not be ideal. In fact, there are many methods and details regarding the use of topical medications.

01 Order of Application

1. When using solutions or suspensions with ointments, apply the solution or suspension first, and wait for it to dry before applying the ointment;

2. When using two or more ointments, they should be applied 1-2 hours apart. Unless otherwise specified, there is no need to wash off the previous ointment;

02 Timing and Frequency of Application

1. Topical solutions and washes are volatile, so they can be applied slightly more frequently, generally every 3 hours;

2. Tinctures and ointments have a longer-lasting effect and can be used 2-3 times a day;

3. Hormonal ointments can generally be applied twice a day, once in the morning and once in the evening.

03 Dosage of Ointments

The amount of ointment used is not about using more, but rather following medical advice and using an appropriate amount. It is not recommended to use cotton swabs to apply ointments, as they can lead to medication loss and reduced efficacy. It is advisable to use the fingertip unit method for application. A fingertip unit refers to the amount of ointment that can cover the tip of an adult’s index finger to the first joint (approximately 2.0-3.0 cm in length), which is about 0.5g of medication, sufficient to cover the skin area of two adult palms. For children, please follow medical advice regarding dosage.

04 Application Method

First, clean the affected area to remove foreign objects and scabs. For skin lesions with hair, trim or remove the hair first. The detailed application methods are as follows:

1. Solutions or suspensions: Mix the suspension well before use. Pour a small amount of the medication into a medicine cup or bottle cap to avoid contaminating the entire bottle. Use a cotton swab to dip into the solution and apply it evenly to the affected area;

2. Ointments: Wash and dry your hands before use. Take an appropriate amount of medication and lightly apply it to the affected area, massaging for 1-2 minutes to rub the medication into the lesion. After application, wash your hands again to remove any medication to avoid contaminating other areas of the skin.

Attention for Those with Sensitive Skin:

When using a new topical medication for the first time, to reduce the risk of allergies, it is recommended to apply a small amount of the medication to a small area of the affected skin or the inner wrist or forearm as a patch test. Observe for 24 hours. If no rash, redness, or itching occurs, you may gradually increase the amount and area of application. If any suspicious reactions occur, stop using the medication immediately and seek medical attention.
