How to distinguish between Wind-Cold and Wind-Heat Cold?

The symptoms of Wind-Cold Cold include: aversion to cold, fever, no sweating, preference for covering with blankets, white tongue coating, clear nasal discharge, and coughing with white phlegm.

The symptoms of Wind-Heat Cold include: fever, no aversion to cold, dry mouth, desire to drink water, yellow and thick tongue coating, yellow purulent nasal discharge, dry stools, hot breath from the nose, coughing with yellow phlegm, and red, swollen sore throat.

Both Wind-Cold and Wind-Heat Cold can present with sore throat. This is because the throat is a crucial defense fortress of the body, a pathway for Qi and blood.

When the body’s surface is invaded by pathogenic factors, Qi and blood rush to the upper jiao and surface to combat the pathogens, leading to temporary congestion in the throat, which manifests as redness, swelling, and pain. Therefore, when cold symptoms are accompanied by throat symptoms, all symptoms should be considered together to determine the nature of the cold.

Although it is often said that cooling herbs for clearing heat and detoxifying should be used cautiously as they can harm the righteous Qi, when diagnosing a heat syndrome as the main issue in a cold, and after excluding the possibility of food stagnation, one should adopt a strategy of using pungent-cool herbs to address the condition. Using ginger soup or scallion soup would be inappropriate.

Commonly used medications for Wind-Heat Cold include:

Yin Qiao Jie Du Wan】, also known as the famous TCM formula Yin Qiao San, which disperses exterior pathogens and clears heat and toxins, suitable for Taiyang and Yangming syndromes.

Children’s Qing Fei Oral Liquid】 is suitable for cough due to external heat. It primarily treats cough associated with Taiyang and Yangming colds.

Bao Ke Ning Granules】 are suitable for cough with nasal discharge and heat syndrome (not due to food stagnation).

Jin Yin Hua Oral Liquid】 is suitable for obvious heat syndrome without food stagnation, especially for sore throat.

Children’s Chi Qiao Qing Re Granules】 are also primarily cooling herbs, with a strong heat-clearing effect, requiring clear evidence of Yangming, Shaoyang, or Taiyang syndromes.

Children’s Ma Gan Granules】 are primarily for Taiyang and Yangming syndromes, relieving cough and phlegm, and benefiting the throat.

Children’s Ke Chuan Ling Oral Liquid】 is derived from the classic formula Ma Xing Shi Gan Tang, suitable for symptoms of exterior cold not yet resolved with internal heat.

Children’s Lung Heat Cough and Wheezing Oral Liquid】 is a Taiyang and Yangming medication, with a strong heat-clearing effect, suitable for cough due to heat cold.
