2. Discussion on the diseases of the organs and meridians: (1) Question: Why does the superior physician treat diseases before they occur? The master said: To treat diseases before they occur, one must recognize the disease of the liver and know that it transmits to the spleen. One should first strengthen the spleen; if the spleen is strong, it will not be affected by evil influences in all four seasons. The intermediate physician does not understand this transmission; when seeing liver disease, they only treat the liver. For liver disease, sour herbs should be used to nourish it, bitter herbs should be used to assist, and sweet herbs should be used to harmonize. Sour enters the liver, bitter enters the heart, and sweet enters the spleen. The spleen can harm the kidneys; if kidney qi is weak, water will not flow; if water does not flow, heart fire will be excessive; if heart fire is excessive, it will harm the lungs; if the lungs are harmed, the metal qi will not flow; if metal qi does not flow, liver qi will be excessive. Therefore, by strengthening the spleen, the liver will heal itself. This is the key to treating the liver by nourishing the spleen. If the liver is deficient, this method should be used; if it is excessive, it should not be used. The classic states: “Deficient and excessive, supplement the insufficient, reduce the excessive,” which is its meaning. The same applies to other organs.(2) Humans are endowed with the five constants and grow according to the wind and qi. Although the wind and qi can give life to all things, they can also harm all things, just as water can float a boat but can also capsize it. If the five organs are unobstructed, the person will be peaceful and harmonious; if external evil winds invade, many will die. There are three main causes of diseases: one is that the meridians are invaded by evil, entering the organs, causing internal issues; two is that the limbs and nine orifices are obstructed, causing external skin issues; three is injuries from rooms, sharp objects, or insects. By detailing this, all diseases can be understood. If a person can nurture caution and prevent evil winds from obstructing the meridians, and if the meridians are not obstructed from transmitting to the organs, then treatment can be administered. If the limbs feel heavy and stagnant, one should practice guiding exercises, breathing techniques, acupuncture, and topical applications, ensuring that the nine orifices are not blocked; furthermore, one should avoid violating the laws of nature and the harm of beasts; rooms should not be exhausted, and food should be consumed in moderation regarding temperature and flavor, ensuring that the body does not weaken, thus preventing diseases from entering the pores. The pores are where the San Jiao connects with the original qi, and the principles are the patterns of the skin and organs.3. Treatment of diseases: (1) For urgent diseases that require internal and external rescue, what does this mean? The master said: For diseases, if the patient has persistent diarrhea and abdominal pain, one should urgently treat the interior; if the body aches and the stool adjusts itself, one should urgently treat the exterior.(2) For chronic diseases combined with acute diseases, one should first treat the acute disease and then the chronic disease.(3) For all diseases in the organs, one should attack them according to their manifestations, such as for thirst, use Zhu Ling Decoction. The rest should be treated similarly.(4) The master said: Each organ disease has its own manifestations; each organ has its own dislikes, and one should treat according to what it does not like. If a patient does not respond to food but instead thinks about it excessively, they will surely develop a fever.4. Treatment of spasms and damp heat diseases: (1) Spasm diseases: 1. Soft spasm: Guo Lou Gui Zhi Decoction. For Tai Yang disease, with symptoms of strong body, pulse is deep and slow, this is a spasm, and Guo Lou Gui Zhi Decoction is the main treatment. 2. Hard spasm: Ge Gen Decoction. For Tai Yang disease, without sweating and with reduced urination, qi rushing to the chest, unable to speak, wanting to have a hard spasm, Ge Gen Decoction is the main treatment. 3. Yang Ming heat causing spasm: Da Cheng Qi Decoction. Spasm is a disease, with fullness in the chest, unable to lie down, legs cramping, and teeth grinding, Da Cheng Qi Decoction can be used.(2) Damp diseases: Treatment principle: induce sweating (mild sweating); promote urination. 1. Cold damp on the surface: Ma Huang Jia Zhu Decoction. For dampness causing body pain, Ma Huang Jia Zhu Decoction can induce sweating appropriately; caution against using fire to attack it. 2. Wind damp on the surface: Ma Huang Xing Ren Yi Yi Ren Decoction. The patient has body pain, fever, and severe symptoms in the afternoon, known as wind damp. This disease is caused by sweating in the wind or prolonged exposure to cold. Ma Huang Xing Ren Yi Yi Ren Decoction can be used (the mechanism is that wind damp combines, wanting to transform into heat). 3. Wind damp combined with qi deficiency: Fang Ji Huang Qi Decoction (both wind damp and wind water). For wind damp, pulse is floating, body heavy, and sweating with aversion to wind, Fang Ji Huang Qi Decoction is the main treatment. 4. Wind damp combined with yang deficiency: ① Exterior yang deficiency: Gui Zhi Fu Zi Decoction, Bai Zhu Fu Zi Decoction. For cold and dampness combining after 8-9 days of cold damage, with body pain and inability to turn over, no vomiting or thirst, pulse is floating, weak, and rough, Gui Zhi Fu Zi Decoction is the main treatment; if the stool is hard and urination is self-beneficial, remove Gui and use Bai Zhu Decoction. ② Exterior and interior yang deficiency: Gan Cao Fu Zi Decoction. For wind damp combining, with joint pain and discomfort, severe pain upon approach, sweating and shortness of breath, urination is not beneficial, aversion to wind and unwillingness to remove clothing, or slight swelling, Gan Cao Fu Zi Decoction is the main treatment.(3) Heat stroke diseases: 1. Heat stroke with excessive heat damaging fluids and qi: Bai Hu Jia Ren Shen Decoction (also treats upper consumption). For Tai Yang heat, this is heat stroke. Sweating with aversion to cold, body heat and thirst, Bai Hu Jia Ren Shen Decoction is the main treatment. (Zhi Mu, Shi Gao, Gan Cao, Jing Mi, Ren Shen; summer heat is predominant, both qi and fluids are damaged without dampness.) 2. Heat stroke with dampness: Yi Wu Gua Ti Decoction. For Tai Yang heat stroke, with body heat and heavy pain, and weak pulse, this is due to cold water damaging the skin. Yi Wu Gua Ti Decoction is the main treatment.5. Treatment of Bai He (Lily) and Hu Huo (Fox) diseases: Bai He disease mechanism: Heart and lung yin deficiency with internal heat. 1. Main formula for Bai He disease: Bai He Di Huang Decoction. For Bai He disease, without vomiting, diarrhea, or sweating, and the disease appears as before, Bai He Di Huang Decoction is the main treatment. 2. Mistaken sweating in Bai He disease: Bai He Zhi Mu Decoction. For Bai He disease, after sweating, Bai He Zhi Mu Decoction is the main treatment. 3. Mistaken diarrhea in Bai He disease: Hua Shi Dai Zhi Decoction. For Bai He disease, after diarrhea, Hua Shi Dai Zhi Decoction is the main treatment. 4. Mistaken vomiting in Bai He disease: Bai He Ji Zi Decoction. For Bai He disease, after vomiting, use the latter formula as the main treatment.(Bai He Ji Zi Decoction) Hu Huo disease main symptoms: Red eyes and erosion of the front and back yin. Internal treatment formula: 1. Gan Cao Xie Xin Decoction. Hu Huo is a disease resembling cold damage, wanting to sleep quietly, unable to close the eyes, restless when lying down, erosion in the throat is a worry, erosion in the yin is a fox, unwilling to eat, averse to food odors, face sometimes red, sometimes black, sometimes white. Touching the upper part causes a loud voice, Gan Cao Xie Xin Decoction is the main treatment (Gan Cao, Huang Qin, Ren Shen, Gan Jiang, Huang Lian, Da Zao, Ban Xia; clears heat, eliminates dampness, supports the righteous and detoxifies). 2. Chi Dou Dang Gui San. External treatment formula: Ku Shen Decoction, Xiong Huang.6. Treatment of malaria diseases: 1. Malaria mother: Refers to malaria that has been prolonged and recurrent, with the righteous qi gradually declining, malaria evil relying on phlegm, forming lumps in the side and causing a disease. Bie Jia Jian Wan formula. For malaria, if it occurs once a month, it should be cured in fifteen days; if not, it should be resolved by the end of the month; if it does not resolve, what should be done? The master said: This is a lump forming a disease, called malaria mother, which should be treated urgently with Bie Jia Jian Wan. (Function: Dissolve lumps, invigorate the righteous, expel evil). 2. Warm malaria: Disease mechanism: Internal heat is excessive, exterior combined with cold evil. Symptoms: No cold but heat, more heat than cold. Bai Hu Jia Gui Zhi Decoction is the main treatment for warm malaria, with pulse being flat, no cold but heat, joint pain, occasional vomiting, Bai Hu Jia Gui Zhi Decoction is the main treatment. 3. Female malaria: (more cold than heat) Shu Qi San formula. For malaria with more cold, it is called female malaria, Shu Qi San is the main treatment.7. Treatment of wind stroke and joint diseases: Wind stroke disease mechanism: Internal deficiency of qi and blood, organ deficiency, external evil inducing joint diseases. Main symptoms of joint disease: Joint changes, swelling, and severe pain. Causes: ① Liver and kidney deficiency, water dampness invading. ② Yin blood deficiency, external wind evil. ③ Qi deficiency from drinking alcohol, sweating in the wind. ④ Stomach has retained heat, externally contracting wind damp. ⑤ Overeating sour and salty foods, injuring the liver and kidneys. 1. Wind damp joint disease (predominantly heat): Gui Zhi Shao Yao Zhi Mu Decoction. For joint pain, body weakness, swollen feet, dizziness, and shortness of breath, Gui Zhi Shao Yao Zhi Mu Decoction is the main treatment. 2. Cold damp joint disease (predominantly cold): Wu Tou Decoction. For joint disease that cannot bend or stretch, with pain, Wu Tou Decoction is the main treatment.8. Treatment of blood bi and deficiency diseases: Blood bi main symptoms: Localized skin numbness without pain, severe cases accompanied by soreness. Severe symptoms: Huang Qi Gui Zhi Wu Wu Decoction. Blood bi with both yin and yang being weak, pulse at the cun position is weak, and the pulse at the chi position is tight, with external symptoms of body numbness, resembling wind bi, Huang Qi Gui Zhi Wu Wu Decoction is the main treatment. (Huang Qi, Gui Zhi, Shao Yao, Sheng Jiang, Da Zao; used to treat qi deficiency and blood stasis, skin numbness and insensitivity). Deficiency diseases: 1. Deficiency of essence: Gui Zhi Jia Long Gu Mu Li Decoction. For loss of essence, with lower abdomen tightness, cold in the yin area, dizziness, weak pulse, and extreme deficiency, for loss of essence, loss of blood, and loss of essence. If the pulse is weak and tight, for men losing essence, women dreaming of intercourse, Gui Zhi Jia Long Gu Mu Li Decoction is the main treatment. (Yin and yang imbalance, the formula harmonizes yin and yang). 2. Deficiency abdominal pain: Xiao Jian Zhong Decoction, Huang Qi Jian Zhong Decoction. For deficiency abdominal urgency, palpitations, bleeding, abdominal pain, dreaming of loss of essence, limbs sore, hands and feet hot, throat dry, mouth dry, Xiao Jian Zhong Decoction is the main treatment. For deficiency abdominal urgency, Huang Qi Jian Zhong Decoction is the main treatment. (Xiao Jian Zhong Decoction is used to treat deficiency spleen deficiency and weak nutrients, abdominal pain, with Gui Zhi Decoction increased with Shao Yao and honey. Huang Qi Jian Zhong Decoction is Xiao Jian Zhong Decoction plus Huang Qi, treating spleen qi deficiency). 3. Deficiency low back pain: Shen Qi Wan. For deficiency low back pain, lower abdomen tightness, and difficulty urinating, Ba Wei Shen Qi Wan is the main treatment. 4. Deficiency causing wind: Shu Yu Wan. For deficiency and various insufficiencies, wind qi causing various diseases, Shu Yu Wan is the main treatment. 5. Deficiency vexation and insomnia: Suan Zao Ren Decoction. For deficiency vexation and inability to sleep, Suan Zao Ren Decoction is the main treatment. 6. Deficiency of dry blood: Da Huang Zhe Chong Wan. For five deficiencies, extreme weakness, abdominal fullness, inability to eat, injury from food, injury from worry, injury from drinking, injury from sexual activity, injury from hunger, injury from labor, injury from meridian qi, with internal dry blood, skin dark on both sides. To nourish the deficiency, Da Huang Zhe Chong Wan is the main treatment.9. Treatment of lung atrophy, lung abscess, cough, and shortness of breath: Lung atrophy (excessive salivation and shortness of breath) Lung qi deficiency and cold: Gan Cao Gan Jiang Decoction. For lung atrophy with phlegm and no cough, the person is not thirsty, must urinate frequently, this is due to the upper deficiency not controlling the lower. This is cold in the lung, with dizziness and excessive salivation, Gan Cao Gan Jiang Decoction is used to warm the lung. Lung abscess: 1. Evil and qi obstruction: Ting Li Da Zao Xie Lung Decoction. For lung abscess, unable to lie down, Ting Li Da Zao Xie Lung Decoction is the main treatment. 2. Blood corruption and pus: Jie Geng Decoction. Coughing with fullness in the chest, cold pulse. Dry throat, not thirsty, occasionally spitting out foul phlegm, for lung abscess, Jie Geng Decoction is the main treatment. Cough and shortness of breath: 1. Cold drink obstructing the lung (asthma): She Gan Ma Huang Decoction. Coughing and shortness of breath, with a sound like a water chicken in the throat, She Gan Ma Huang Decoction is the main treatment. 2. Phlegm obstruction in the lung: Zao Jiao Wan. Coughing with reverse shortness of breath, occasionally spitting out phlegm, but unable to sleep, Zao Jiao Wan is the main treatment. 3. Heat drink obstructing the lung: Yue Bi Jia Ban Xia Decoction. Coughing and shortness of breath, this is lung distension, the person is wheezing, eyes like they are falling out, pulse is floating and large, Yue Bi Jia Ban Xia Decoction is the main treatment. 4. Cold drink with heat: Hou Po Ma Huang Decoction, Ze Qi Decoction, Xiao Qing Long Jia Shi Gao Decoction. Coughing with floating pulse, Hou Po Ma Huang Decoction is the main treatment. For deep pulse, Ze Qi Decoction is the main treatment. Lung distension, coughing and shortness of breath, restlessness and wheezing, floating pulse, with water under the heart, Xiao Qing Long Jia Shi Gao Decoction is the main treatment. 5. Lung and stomach yin deficiency with qi reversal: Mai Men Dong Decoction. For severe qi reversal, throat discomfort, stopping qi reversal, Mai Men Dong Decoction is the main treatment.10. Treatment of chest obstruction, heart pain, and shortness of breath: Chest obstruction: 1. Typical symptoms (wheezing, coughing, sputum, chest and back pain, shortness of breath): Guo Lou Xie Bai Bai Jiu Decoction. For chest obstruction disease, wheezing, coughing, chest and back pain, shortness of breath, pulse at the cun position is deep and slow, pulse at the guan position is tight and rapid, Guo Lou Xie Bai Bai Jiu Decoction is the main treatment. 2. Phlegm and drink obstruction: Guo Lou Xie Bai Ban Xia Decoction. For chest obstruction unable to lie down, heart pain penetrating the back, Guo Lou Xie Bai Ban Xia Decoction is the main treatment. 3. Predominantly deficient and predominantly excessive: Zhi Shi Xie Bai Gui Zhi Decoction, Ren Shen Decoction. For chest obstruction with fullness in the heart, gas trapped in the chest, chest fullness, and reverse qi in the side, Zhi Shi Xie Bai Gui Zhi Decoction is the main treatment; Ren Shen Decoction is also the main treatment. 4. Drink obstructing qi stagnation: Fu Ling Xing Ren Gan Cao Decoction, Ju Zhi Jiang Decoction. For chest obstruction, gas blockage in the chest, shortness of breath, Fu Ling Xing Ren Gan Cao Decoction is the main treatment; Ju Zhi Jiang Decoction is the main treatment. 5. Chest obstruction acute symptoms: Yi Yi Ren Fu Zi San. For chest obstruction with slow and acute symptoms, Yi Yi Ren Fu Zi San is the main treatment.11. Treatment of abdominal fullness, cold hernia, and food retention: Abdominal fullness: 1. Interior excess combined with exterior cold: Hou Po Qi Wu Decoction. For abdominal fullness, fever for ten days, pulse is floating and rapid, diet remains the same, Hou Po Qi Wu Decoction is the main treatment. 2. Interior excess combined with Shao Yang: Da Chai Hu Decoction. If pressing the heart causes fullness and pain, this is excess, and it should be purged, Da Chai Hu Decoction is appropriate. 3. Interior excess with heavy accumulation: Hou Po San Wu Decoction. For pain and closure, Hou Po San Wu Decoction is the main treatment. 4. Interior excess with accumulation and heaviness: Da Cheng Qi Decoction. For abdominal fullness that does not decrease, and insufficient reduction, it must be purged, Da Cheng Qi Decoction is the main treatment. 5. Deficiency cold drink reversal: Fu Zi Jing Mi Decoction, Chi Wan. For abdominal cold qi, thunder-like pain, chest and side fullness, vomiting, Fu Zi Jing Mi Decoction is the main treatment. Cold qi reversal, Chi Wan is the main treatment. 6. Spleen and stomach deficiency cold: Da Jian Zhong Decoction. For severe cold pain in the heart and chest, unable to eat, abdominal pain, rising to the skin, with head and feet pain and unable to touch, Da Jian Zhong Decoction is the main treatment. 7. Cold excess internal accumulation: Da Huang Fu Zi Decoction. For pain under the side, fever, and tight pulse, this is cold, and it should be treated with warming medicine, Da Huang Fu Zi Decoction is the main treatment.12. Treatment of five organs, wind, cold, and accumulation diseases: 1. Liver accumulation: Xuan Fu Hua Decoction. For liver accumulation, the person often wants to step on the chest, before suffering, but wants to drink hot, Xuan Fu Hua Decoction is the main treatment. 2. Kidney accumulation: Gan Cao Gan Jiang Fu Ling Bai Zhu Decoction. For kidney accumulation disease, the person feels heavy, cold in the waist, as if sitting in water, appearing water-like, not thirsty, urination is self-beneficial, diet remains the same, the disease belongs to the lower jiao, with labor sweating, clothing damp, obtained for a long time, cold pain below the waist. Abdominal heaviness like carrying five thousand coins, Gan Jiang Ling Bai Zhu Decoction is the main treatment.13. Treatment of phlegm, drink, and cough diseases: Treatment principle: For phlegm and drink diseases, one should use warming medicine to harmonize. Four types of drinks: If the person was previously robust and is now thin, water flows in the intestines, making a dripping sound, this is phlegm drink; if after drinking, water flows under the side, coughing and pain, this is suspended drink; if water flows and returns to the limbs, it should sweat out but does not, and the body feels heavy, this is overflow drink; if coughing and leaning, shortness of breath unable to lie down, the shape is like swelling, this is support drink. Treatment principle: For phlegm and drink diseases, one should use warming medicine to harmonize. 1. Drink stopping in the heart and intestines: ① Spleen and stomach yang deficiency drink stopping: Fu Ling Gui Zhi Bai Zhu Gan Cao Decoction. For phlegm and drink in the heart, fullness in the chest and side, dizziness, Fu Ling Gui Zhi Bai Zhu Gan Cao Decoction is the main treatment. ② Water drink rising and causing dizziness: Ze Xie Decoction. For phlegm and drink in the heart, the person suffers from dizziness, Ze Xie Decoction is the main treatment. ③ Stopping drink rising and causing vomiting: Xiao Ban Xia Decoction. For support drink unable to rest, Ting Li Da Zao Xie Lung Decoction. The formula is seen in lung abscess with vomiting, thirst is for wanting to resolve, now not thirsty, due to phlegm and drink in the heart, Xiao Ban Xia Decoction is the main treatment. ④ Lower jiao drink rising and causing palpitations: Wu Ling San. If a thin person has palpitations below the navel, vomiting phlegm and foam, this is water, Wu Ling San is the main treatment. ⑤ Intestinal drink forming solid: Ji Jiao Li Huang Wan. For abdominal fullness, dry mouth and tongue, this is water gas in the intestines, Ji Jiao Li Huang Wan is the main treatment. ⑥ Retained drink evil wanting to be expelled: Gan Sui Ban Xia Decoction. The patient has a hidden pulse, the person wants to self-benefit, but the benefit is quick, even if it benefits, the heart remains hard and full, this is retained drink wanting to be expelled, Gan Sui Ban Xia Decoction is the main treatment.14. Treatment of diabetes, urinary difficulties, and dysuria: Diabetes: 1. Excessive heat damaging qi and fluids: Bai Hu Jia Ren Shen Decoction. For thirst wanting to drink water, dry mouth and tongue, Bai Hu Jia Ren Shen Decoction is the main treatment. 2. Kidney qi deficiency: Shen Qi Wan. For men with diabetes, urinating frequently, drinking one dou, urinating one dou, Shen Qi Wan is the main treatment. Urinary difficulties, dysuria: 1. Bladder qi transformation not functioning: Wu Ling San. Pulse is floating, urinary difficulties, slight heat and thirst, should promote urination and induce sweating, Wu Ling San is the main treatment. Thirst wanting to drink water, if water enters, it vomits, this is called water reversal, Wu Ling San is the main treatment. 2. Upper dryness and lower cold water stopping: Guo Lou Qu Mai Wan. For urinary difficulties, if there is water gas, the person seems thirsty, Guo Lou Qu Mai Wan is the main treatment. 3. Water heat mutual binding damaging yin: Zhu Ling Decoction. Pulse is floating and feverish, thirst wanting to drink water, urinary difficulties, Zhu Ling Decoction is the main treatment.15. Treatment of water diseases: Treatment method: ① Promote urination (below the waist), induce sweating (above the waist) ② Attack water and expel water. Wind water: 1. Wind water with surface deficiency: Fang Ji Huang Qi Decoction. Wind water, pulse is floating, body heavy, sweating with aversion to wind, Fang Ji Huang Qi Decoction is the main treatment. If there is abdominal pain, add Shao Yao. 2. Wind water with heat: Yue Bi Decoction. Wind water with aversion to wind, the whole body is swollen, pulse is floating and not thirsty, continues to sweat, without great heat, Yue Bi Decoction is the main treatment. Skin water: 1. Skin water with heat: Yue Bi Jia Zhu Decoction. For internal water, the whole body and face are yellow and swollen, pulse is deep, urinary difficulties, this causes the disease water. If urination is self-beneficial, this is loss of fluids, hence thirst, Yue Bi Jia Zhu Decoction is the main treatment. 2. Skin water with surface excess: Gan Cao Ma Huang Decoction. For internal water, Yue Bi Jia Zhu Decoction is the main treatment, Gan Cao Ma Huang Decoction is also the main treatment. 3. Skin water with qi deficiency and yang obstruction: Fang Ji Fu Ling Decoction. Skin water as a disease, with swelling in the limbs, water gas in the skin, limbs moving slightly, Fang Ji Fu Ling Decoction is the main treatment. 4. Skin water with damp heat internally obstructing: Pu Hui San. For skin water, Pu Hui San is the main treatment.16. Treatment of jaundice diseases: Disease mechanism: Spleen color must be yellow, with heat and stasis. 1. Guo Jaundice: Yin Chen Hao Decoction. Guo jaundice as a disease, with cold and heat not eating, eating causes dizziness, discomfort in the chest, and prolonged yellowing, Yin Chen Hao Decoction is the main treatment. 2. Alcohol jaundice: Zhi Zi Da Huang Decoction. Alcohol jaundice, with discomfort in the heart or heat pain, Zhi Zi Da Huang Decoction is the main treatment. 3. Female labor jaundice: Nao Shi Fan Shi San. Yellowing occurring in the afternoon, with aversion to cold, this is caused by female labor. Bladder urgency, fullness in the lower abdomen, yellowing all over, black on the forehead, heat below the feet, causing black jaundice. If the abdomen is swollen like water, the stool must be black, sometimes loose, this is the disease of female labor, not water, abdominal fullness is difficult to treat, Nao Shi Fan Shi San is the main treatment. 4. Jaundice combined with Shao Yang syndrome: Chai Hu Decoction. For all yellows, with abdominal pain and vomiting, Chai Hu Decoction is appropriate. 5. Jaundice mistakenly treated causing hiccups: Xiao Ban Xia Decoction.17.
