Thirty-fourth Damp-Heat Syndrome: For seven to eight days, there is no thirst, the voice is silent, and urination is normal, with no aversion to food. The patient remains silent, consciousness clouded, and treatments with pungent, aromatic herbs for cooling and dispersing are ineffective; this indicates the presence of evil. The hand Jueyin (Pericardium) is affected, with both the host and guest being involved. It is advisable to use Wu Youke’s Sanjia San formula: Zui Di Bie Chong (drunken soft-shelled turtle), vinegar-fried turtle shell, earth-fried pangolin, and dead silkworms, along with Chai Hu (Bupleurum), and Tao Ren (Peach Kernel) paste, among other ingredients.
Self-annotation: Summer dampness first injures the Yang aspect, but if the illness persists without resolution, it must affect the Yin, leading to both Yin and Yang being constrained, causing Qi stagnation and blood stasis, preventing summer dampness from being released.
Xiong’s commentary: According to Zhang, this is Xue’s self-annotation, yet it combines the two Qi of summer heat and dampness to explain damp-heat syndromes. If it is said that dampness exists within summer heat, then what is the dampness that descends from summer? A smile. I fear that future learners may be confused, hence I do not hesitate to elaborate. Thus, it deeply penetrates Jueyin, causing the meridians to coagulate and stagnate, preventing the Yang from generating vitality. It cannot sprout; the vitality descends without rising, the heart obstructs, and the spiritual energy is not flowing, resulting in confusion and silence. Breaking stagnation and unblocking stasis will allow the meridians to flow and the evil to be resolved.
Hai Chang Xu Yizhai states: This section is akin to the cold damage category of the hundred lily disease; Zhao Yide, Zhang Luyu, and Tao Nongtang consider it a heart disease. Xu Zhong can attribute it to lung disease. This discussion also presents Jueyin treatment methods, as all hundred pulses are one family, addressing the illness where the original Qi is not distributed, and the evil Qi lingers. Thus, following the methods of the ancestor Zhang Zhongjing, using different types of dynamic substances, turtle shell enters Jueyin, and Chai Hu guides it, allowing the evil within the Yin to reach the surface. Insects entering the blood are guided by Tao Ren to allow the evil in the blood to be fully discharged. The pangolin enters the meridians, and the dead silkworm guides it, allowing the evil in the meridians to disperse through wind. Due to the prolonged illness, Qi stagnation and blood stasis cannot be cured by rigid adherence to established methods.
Wang’s commentary: This has the symptom of clouded consciousness, indicating it is not the hundred lily disease, thus differing from it; treating the hundred lily disease should focus on the lungs.
Qinglong’s summary: If damp-heat Qi does not resolve, it will inevitably enter the blood aspect. Self-annotation Shou Jueyin Xinbao (Hand Jueyin Pericardium) indicates that it is indeed Qi and blood stagnation obstructing Jueyin, affecting consciousness. Cooling and dispersing pungent herbs, along with aromatic herbs for eliminating turbidities, are ineffective; this evil enters the blood aspect, hence the ineffectiveness! Chai Hu guides it into the liver channel, while Tu Yuan, turtle shell, and pangolin insects possess powerful dynamic dispersing abilities, combined with Tao Ren to break blood stasis, and dead silkworms to transform phlegm and disperse stasis. The entire formula works to invigorate blood, dispel stasis, transform phlegm, and disperse stasis with great force! He Lianchen also suggested using An Gong Niu Huang and other awakening products for reference!
