Bingpeng San: A Traditional Chinese Medicine Powder for Heat Detoxification

Bingpeng San originates from the Ming Dynasty book Wai Ke Zheng Zong, and is a topical preparation in Traditional Chinese Medicine (TCM) composed of Bing Pian (Borneol), Peng Sha (Borax), Zhu Sha (Cinnabar), and Xuan Ming Fen (Calamine). Each bottle contains 0.6 grams and is traditionally used in TCM for its effects of clearing heat and detoxifying, reducing swelling and alleviating pain, particularly for oral diseases such as sore throat, swollen gums, and mouth ulcers caused by heat toxin accumulation.

Chen Shigong, a physician from the Ming Dynasty (1555-1636), was known for his extensive study of medicine from a young age, particularly focusing on surgical techniques. He categorized various surgical conditions and authored Wai Ke Zheng Zong, advocating for a combined approach of surgery and medication. Wai Ke Zheng Zong is a specialized surgical text compiled by Chen Shigong, completed in 1617. The book consists of four volumes, with the first volume discussing the etiology, diagnosis, and treatment of surgical diseases, while the subsequent volumes cover over one hundred common surgical conditions, detailing their causes, clinical manifestations, treatment methods, and including typical case studies. The book features over thirty illustrations depicting the locations and shapes of various important sores and swellings, and concludes with methods for refining various medicines. It is renowned in TCM surgical literature for its detailed symptom classification and precise treatment discussions, making it a highly respected reference for the study and practice of TCM surgery.

【Formula】 Bing Pian 50g, Peng Sha (calcined) 500g, Zhu Sha 60g, Xuan Ming Fen 500g.

【Preparation】 The above four ingredients are ground into a fine powder, with Zhu Sha being water-milled into an extremely fine powder, and Peng Sha (calcined) crushed into a fine powder. Bing Pian is finely ground and mixed with the above powders and Xuan Ming Fen, then sieved and blended to obtain the final product.

【Formula Analysis】

The formula includes Bing Pian, which disperses and cools, has a fragrant aroma, and is effective in clearing heat, reducing fire, alleviating swelling, and promoting tissue regeneration, thus serving as the monarch drug. Peng Sha clears heat and detoxifies, prevents infection, and promotes tissue regeneration, enhancing the monarch drug’s effects, thus serving as the minister drug. Zhu Sha effectively alleviates toxic swelling and pain, while Xuan Ming Fen clears heat and reduces swelling. Together, these two serve as assistant drugs. The combination of all ingredients works synergistically to clear heat, detoxify, reduce swelling, and alleviate pain.

【Clinical Applications】

1. Throat obstruction due to heat toxin accumulation, causing fire to scorch the throat. Symptoms include redness and swelling of the throat, pain, difficulty swallowing, dry mouth, yellow urine, constipation, red tongue with yellow coating, and rapid pulse; applicable for acute pharyngitis with these symptoms.

2. Dental pain due to excessive stomach heat, leading to upward attack along the meridians. Symptoms include red and swollen painful gums, thirst, constipation, red tongue with yellow coating, and rapid pulse; applicable for periodontal disease with these symptoms.

3. Mouth ulcers due to heat toxin accumulation, causing fire to attack. Symptoms include ulcers on the tongue and mouth, burning pain, irritability, insomnia, constipation, red tongue with yellow coating, and rapid pulse; applicable for oral inflammation and ulcers with these symptoms.

Additionally, this product has been reported to treat fungal vaginitis and cervical erosion caused by heat toxin and dampness.

【Pharmacology and Toxicology】

This product exhibits anti-ulcer, analgesic, anti-inflammatory, and antibacterial effects.

1. Anti-ulcer: This product has therapeutic effects on oral mucosal ulcers induced by mechanical injury and continuous infection with Staphylococcus aureus in rabbits, and promotes healing of experimental oral mucosal ulcers in guinea pigs.

2. Analgesic: This product, when administered subcutaneously or orally as a suspension, reduces the number of writhes in mice induced by intraperitoneal injection of acetic acid.

3. Anti-inflammatory: This product inhibits ear swelling in mice caused by xylene and foot swelling in rats induced by egg white.

4. Antibacterial: In vitro tests show this product inhibits Staphylococcus aureus, Escherichia coli, diphtheria bacillus, and catarrhal bacillus.
