Relieving Rhinitis and Sinusitis with Traditional Chinese Medicine: A Case Study

Once, a patient questioned me: “What does TCM have to do with rhinitis and the spleen and stomach?” This question was like a thorn, but it awakened me: as a doctor, I must rely on my conscience and let the results speak.

My clinical case:

A 5-year-old child with rhinitis and adenoid hypertrophy had tried various Western medications and antihistamines without improvement. The child’s mother was very distressed, saying the child couldn’t sleep at night due to nasal congestion, and when lying down, mucus would flow back, causing headaches. The child’s face was pale, and dark circles under the eyes were alarming. Upon examining the tongue coating, it was almost invisible.

The mother mentioned that the child had always had loose stools and poor appetite, and rhinitis would flare up with every cold.

I considered that this child had Qi deficiency of the spleen and stomach, with phlegm-damp obstructing the lungs, leading to rhinitis and adenoid hypertrophy.

I told the mother that we needed to address the spleen and stomach. She was taken aback: “How can TCM connect rhinitis with the spleen and stomach?”

I explained, children are inherently spleen-deficient, and since the spleen is the foundation of postnatal life and the source of Qi and blood, it is also the mother of the lungs. If the spleen and stomach are weak, they cannot nourish the lungs adequately, leading to lung Qi deficiency.

The lungs open to the nose; if lung Qi is insufficient, the nose cannot resist external pathogens, making rhinitis more likely.

I suggested we try this formula and see the results. The parents agreed after some consideration.

Formula: Dang Shen (Codonopsis pilosula) and Chao Bai Zhu (Atractylodes macrocephala) each 10g, Fu Ling (Poria cocos) 15g, Zhi Gan Cao (Honey-fried Licorice) 6g, Chen Pi (Aged Tangerine Peel), Mu Xiang (Aucklandia), and Sha Ren (Amomum fruit) each 5g, Jiang Ban Xia (Pinellia ternata) 9g, Xin Yi Hua (Magnolia flower) and Cang Er Zi (Xanthium) each 6g. (The formula can be adjusted according to symptoms; if symptoms match, please consult a TCM practitioner for further adjustments, do not self-prescribe.)

Topical application of Bi Yan Ning Nasal Oil.

This formula is modified from Xiang Sha Liu Jun Wan, very gentle, capable of treating rhinitis and addressing spleen deficiency and phlegm-damp.

A white and greasy tongue coating indicates excessive dampness. Those with spleen deficiency tend to have more dampness. The spleen is the body’s cleaner; when it is deficient, the cleaner goes on strike, and dampness cannot be expelled. Therefore, while tonifying the spleen, we must first eliminate dampness.

The approach is simple: tonify Qi, strengthen the spleen, transform phlegm, and eliminate dampness to ensure smooth lung Qi.

What were the results?

After 7 doses of medicine, during the follow-up, the child’s nasal congestion and runny nose improved significantly, headaches resolved, appetite increased, and stools became formed. After taking another 14 doses, rhinitis was almost gone, the child was energetic, tongue coating normalized, and pulse was strong. Since then, there have been no further issues.

Seeing the child so well made me truly happy. Throughout my years as a doctor, I aim for practicality, to treat and save lives, to be true to my conscience, and to spare children from suffering and parents from worry.
