Clinical Observation of the Efficacy of Chuan Huang Oral Liquid Combined with Lactulose in Treating Deficiency of Body Fluids and Blood in 40 Cases of Constipation Type Irritable Bowel Syndrome

Constipation is a common clinical condition with diverse causes. In clinical practice, it has been found that Chuan Huang Oral Liquid combined with Lactulose has a significant therapeutic effect on constipation due to deficiency of body fluids and blood.

Report as follows:1 Clinical Data 1.1 Inclusion Criteria. The cases were outpatients from our hospital between January 2015 and October 2016, totaling 40 cases, with Western medical diagnosis conforming to constipation type irritable bowel syndrome, and TCM diagnosis conforming to deficiency of body fluids and blood type constipation.

1.1.1 Western Medical Diagnostic Criteria: Refer to the “Consensus on the Diagnosis and Treatment of Irritable Bowel Syndrome (2007, Changsha)” [2] Diagnosis (1) Recurrent abdominal pain or discomfort lasting more than 3 months, with at least 2 of the following: ① Symptoms improve after defecation; ② Change in frequency of defecation; ③ Change in stool characteristics (appearance). (2) The following laboratory tests are all normal: ① Routine blood and urine tests; ② Routine stool tests, occult blood, and cultures; ③ Thyroid function tests; ④ Ultrasound examination of the liver, gallbladder, pancreas, and spleen; ⑤ Erythrocyte sedimentation rate. (3) No significant abnormalities found in colonoscopy.

1.1.2 TCM Diagnostic Criteria Reference:“Consensus on the TCM Diagnosis and Treatment of Irritable Bowel Syndrome”[3] Diagnosis of constipation due to deficiency of body fluids and blood. Main symptoms include dry and hard stools, stools resembling sheep droppings, red tongue with little coating or pale tongue with white coating, dry mouth with little fluid, dizziness, tinnitus, weakness in the lower back and knees, palpitations, flushed cheeks, and weak pulse.

1.1.3 Exclusion Criteria:① Pregnant women and children; ② Those unable to cooperate with treatment. 1.2 General Information. The 40 patients were randomly divided into treatment and control groups based on the order of their visits, with 20 cases in each group. No cases dropped out during the experiment. In the treatment group, there were 9 males and 11 females; the course of disease ranged from 1 to 10 years, with an average of (5.46±11.9) years; ages ranged from 27 to 67 years, with an average of (49.2±9.1) years. In the control group, there were 8 males and 12 females; the course of disease ranged from 1 to 15 years, with an average of (3.99±0.94) years; ages ranged from 31 to 70 years, with an average of (52.7±8.9) years. Statistical analysis showed no significant differences in baseline characteristics between the two groups (P>0.05), indicating comparability. 2 Treatment Methods Both groups took Lactulose (produced by Hainan Puli Pharmaceutical Co., Ltd.) 30ml, once daily; the treatment group also took Chuan Huang Oral Liquid (produced by Sichuan Shenghe Pharmaceutical Co., Ltd.) 10ml, twice daily. The treatment lasted for 4 weeks.

3 Efficacy Observation:3.1 Efficacy Evaluation Criteria. Cure: Normal bowel movements, all other symptoms disappear. Significant effect: Constipation significantly improved, interval time and stool quality approaching normal, most other symptoms disappear. Effective: Interval time for bowel movements shortened by 1 day, or stool quality remains hard with improvement in other symptoms. Ineffective: No improvement in constipation or other symptoms. Statistical methods were analyzed using SPSS statistical software, with data described as ± standard deviation, and measurement data analyzed using t-test, while count data analyzed using X2 test.

3.2 Treatment Results. Comparison of efficacy between the two groups after treatment (P<0.05) indicated that the treatment group was superior to the control group, with both groups showing significant improvement in clinical symptoms. The combination of Chuan Huang Oral Liquid and Lactulose significantly alleviated constipation with remarkable therapeutic effects. Health Way, November 2016, Volume 15, No. 1154 indicates that compared to the control group, P<0.05. The treatment group had a significantly shorter onset time than the control group, with the total effective rate weekly comparison showing the treatment group superior to the control group (P<0.05). The combination of Chuan Huang Oral Liquid and Lactulose for treating constipation has a faster onset time and significant efficacy.

4 Discussion“Revised Yan’s Jisheng Formula – Treatment of Constipation” states, “Moreover, sweating and promoting urination, and women after childbirth losing blood, depleting body fluids, often lead to constipation.” “Jingyue Quanshu – Constipation” states, “Constipation occurs in the elderly, those with deficiency, those with yin organ issues, and after childbirth, illness, excessive sweating, or excessive urination, or after significant blood loss, often leading to dryness and constipation. This is not only due to deficiency of qi and blood but also due to depletion of body fluids.” Constipation due to deficiency of body fluids and blood is common in clinical practice. Lactulose has few adverse reactions and is widely used clinically, making it a well-tolerated and safe laxative [4]. Chuan Huang Oral Liquid contains ingredients such as Danshen (Salvia miltiorrhiza), Danggui (Angelica sinensis), Zhi He Shou Wu (Processed Polygonum multiflorum), Gou Qi Zi (Lycium barbarum), Dang Shen (Codonopsis pilosula), Huang Qi (Astragalus membranaceus), Qi She (Agkistrodon), Chuan Xiong (Ligusticum chuanxiong), Du Zhong (Eucommia ulmoides), Ge Jie (Gecko), and Hai Long (Sea Dragon), which have functions of tonifying qi and nourishing blood, nourishing liver and kidney, and promoting blood circulation and removing stasis. Chuan Huang Oral Liquid contains five effective components including catechin, astragaloside, and others [5]. As a traditional Chinese medicine, Chuan Huang Oral Liquid not only leverages the advantages of TCM but is also convenient to take. The combination of Lactulose and Chuan Huang Oral Liquid for treating constipation due to deficiency of body fluids and blood type irritable bowel syndrome shows good efficacy and highlights the characteristics and advantages of TCM [6]. It is worth promoting in clinical practice.
