Traditional Chinese Medicine Approaches to Seasonal External Pathogenic Diseases in Winter and Spring

A five-year-old child recently began experiencing fever at midnight, but was in good spirits and showed no other discomfort. The family administered antipyretic granules, warm baths, and applied cooling patches at home; the next day, the child’s temperature returned to normal, but rose again to over 38.5°C at midnight, returning to normal by morning. The child showed no appetite and occasionally had slight abdominal discomfort, but overall was in good spirits and did not regularly take antipyretic granules. On the third day, the child woke up with nasal congestion and throat discomfort, leading them to visit the TCM outpatient clinic at Hunan Wangwang Hospital. Dr. Pan Yiping, the attending physician, diagnosed the child with an adenovirus infection after ruling out other conditions, and prescribed a Chinese herbal decoction to disperse wind and relieve the exterior, as well as clear heat and detoxify.

That evening, the child began taking the herbal decoction once, and their temperature continued to rise to around 38.4°C for about two hours. After sweating slightly, their temperature returned to normal. During the day, the throat discomfort and nasal congestion improved, and no further temperature rise was observed. The child continued taking the herbal decoction for one more day.

On the fifth day, during a follow-up at the TCM outpatient clinic, the child’s nasal congestion and throat discomfort had largely improved, but they began to experience a runny nose and cold hands and feet. At night, the child had a rattling sound in their throat while sleeping. Dr. Pan adjusted the herbal decoction to warm the lungs, transform phlegm, and descend qi, and advised the child to soak their feet in warm water before bed to alleviate the coldness in their hands and feet. After one dose of the decoction, the runny nose and nighttime rattling improved, and after two doses, the symptoms were relieved.
