There are many methods of diagnosis in Traditional Chinese Medicine (TCM), such as the Eight Principles (Ba Gang) diagnosis, Zang-Fu diagnosis, Six Meridians diagnosis, Qi-Blood-Jin-Fluid diagnosis, and Wei-Qi-Ying-Blood diagnosis; however, the most important is the Eight Principles diagnosis, as it is the foundation of all diagnostic methods and serves as a guideline.

The Eight Principles refer to: Exterior (Biao), Interior (Li), Cold (Han), Heat (Re), Deficiency (Xu), Excess (Shi), Yin (Yin), and Yang (Yang).

The Eight Principles provide a comprehensive discussion on the nature, location, and the rise and fall of pathogenic factors in diseases.

If the disease is not located in the exterior, it belongs to the interior; if the nature of the disease is not heat, it belongs to cold; if the pathogenic factor is not deficient, it is excess. Among these, exterior and heat belong to Yang; interior, deficiency, and cold belong to Yin.

Thus, despite the numerous types of diseases and the ever-changing symptoms, the overall response of the human body can be summarized as Yang syndrome and Yin syndrome; therefore, Yin and Yang are the overarching principles and essence.

Below is a brief description of the Eight Principles diagnosis:

(1) Exterior and Interior

Exterior and interior indicate the location of the disease and the severity of the condition. If the disease is in the skin and muscles, it is an exterior syndrome; if it is in the Zang-Fu organs, it is an interior syndrome. When an external pathogen invades the body, it first manifests on the skin and meridians, indicating an exterior disease; if the pathogen enters the interior or arises from within, it causes symptoms in the Zang-Fu organs and Qi-Blood, indicating an interior disease. Exterior diseases are generally mild, while interior diseases are more severe. Differentiating between exterior and interior syndromes cannot be solely based on anatomical locations of the disease; it is more important to distinguish based on the characteristics of the symptoms.

For example, exterior syndrome: fever, aversion to cold (or wind), headache, body aches, soreness in the limbs, nasal congestion, thin white tongue coating, floating pulse, with fever, aversion to cold (or wind), and floating pulse being the main characteristics of exterior syndrome. The saying in TCM, “For every bit of aversion to cold, there is a bit of exterior syndrome,” holds true.

For example, interior syndrome: Interior syndrome is the opposite of exterior syndrome; it is not characterized by fever, aversion to cold, and floating pulse appearing together, but rather by heat without cold, or cold without heat, and a pulse that is not floating. The specific location of the interior syndrome should be diagnosed in conjunction with the symptoms of the Zang-Fu organs.

(2) Cold and Heat

Cold and heat indicate the nature of the disease, which is a specific manifestation of the excess or deficiency of Yin and Yang. “Excess Yang leads to heat, excess Yin leads to cold.”

For example, cold syndrome: Any symptoms caused by cold pathogens or due to a decline in the body’s metabolic functions belong to cold syndrome. Generally, it can be divided into exterior cold syndrome and interior cold syndrome.

Exterior cold syndrome typically presents with mild fever, severe aversion to cold (or wind), and a floating tight or floating slow pulse. At this time, one should release the exterior and disperse the cold. Common formulas may include 80•260•50 or 50•60•80 or 260•5000, etc.

Interior cold syndrome presents with no fever, aversion to cold, a preference for warm drinks, cold hands and feet, pale complexion, clear and long urination, loose stools, pale tongue, white coating, and a slow pulse. The corresponding formulas may include 650•3820, or 380•650, or 80•70•2650, etc., to warm the interior and disperse the cold.

For example, heat syndrome: Any symptoms caused by heat pathogens or due to an excess of the body’s metabolic functions belong to heat syndrome. Generally, it can be divided into exterior heat syndrome and interior heat syndrome.

Exterior heat syndrome typically presents with severe fever, mild aversion to cold (or wind), dry mouth and throat, red tongue, thin white coating, and a rapid floating pulse. At this time, one should clear heat, disperse wind, and release the exterior. Common formulas may include 050•00300•020 or 00500•060•020 or 020•060•00500 or 00600•050.

Interior heat syndrome presents with fever or tidal fever, or five hearts heat, no aversion to cold but aversion to heat, thirst with yellow urine, red tongue, and rapid pulse. The corresponding formulas may include 00600•00400•00300 or 003•006•00500 or 001000•06500•0200, etc., to clear heat, eliminate doubts, nourish Yin, and calm the spirit.

(3) Deficiency and Excess

Generally, deficiency and excess can be distinguished based on exterior and interior.

Exterior deficiency syndrome: Commonly presents with fever, sweating, aversion to wind, and a floating slow pulse; at this time, the main treatment is to support the righteous Qi and release the exterior; corresponding formulas may include 2650•80•70, or 650•820, or 2000•650•70, etc.

Exterior excess syndrome: Commonly presents with fever, aversion to cold or wind, no sweating, and a floating tight pulse. Corresponding formulas may include 080•020•650; 260.40.500 or 720.6400, etc., to release the exterior, clear heat, and disperse wind.

Interior deficiency syndrome: The range of interior deficiency is broad, including Qi deficiency, blood deficiency, Yin deficiency, Yang deficiency, and various Zang-Fu organ functional insufficiencies, all of which can be termed interior deficiency. Formulas can be tailored based on different conditions. For Qi deficiency, corresponding formulas may include 720•650, or 2000•650; for blood deficiency, formulas may include 400•300•880 or 4300•260•800, or formulas like 2600.4500, etc.; for Yin deficiency, formulas may include 00600•00400•700 or 600•5400•70, etc.; for Yang deficiency, formulas may include 650.380.20 or 05000.60.4380, etc.; in summary, deficiencies should be supplemented.

Interior excess syndrome: Any symptoms of constipation, abdominal pain with resistance to pressure, and a strong pulse can be considered as interior excess syndrome. Interior excess syndrome can also be divided into cold and heat. If accompanied by fever, flushed face, restlessness, thirst, short red urine, and a strong pulse, it is an excess heat syndrome. Corresponding formulas may include 003•006 or 006•004•00100, or 00300•00600•00400, etc. If accompanied by cold limbs, preference for warmth, no thirst, no yellow urine, white coating, and a strong slow pulse, it is an interior cold excess syndrome. Corresponding formulas may include 650.3820 or 650.380.260 or 650.30.820, etc.

In summary, one should “treat cold with heat” and “treat heat with cold”; for deficiencies in Yin and Yang, one should supplement Yin or Yang (as illustrated above).

(4) Yin and Yang

This brings us to Traditional Chinese Medicine.TCM and Western medicine belong to two different systems; Western medicine is empirical science, with all understanding based on precise logic and experimental results, employing methods of “dissection, analysis, and reduction”;whereas TCM is different, as it establishes a health system through inductive reasoning and summary, using methods of “analogy, trial and error, and synthesis.” It is meaningless to distinguish which is superior or inferior, as the two systems approach the study of the human body from different perspectives and employ different methods.

The foundation of TCM is also built upon the Book of Changes. The Book of Changes uses holistic and diagnostic thinking to study the human body, providing examples:in summer, the fire attribute is strong, and fire can overcome metal; thus, a person born in this season has a weaker metal attribute, which corresponds to the lungs, large intestine, skin, and spine, making them susceptible to diseases in these areas.Moreover, metal generates water, which corresponds to the kidneys and the reproductive and urinary systems; however, if metal is weak, it cannot generate water, leading to potential kidney issues—this is a horizontal perspective.Each year, the relative positions of celestial bodies change, thus affecting the magnetic field properties acting on the Earth, which means the attributes of the five elements also change; if a year has a weak metal attribute and a strong fire attribute, then a person with originally weak metal may fall ill—this is a vertical perspective.

The correct application of zero is the “good prescription” in symbolic number therapy formulas.The Book of Changes and TCM believe: “The Book of Changes emphasizes the circular principle; currently, some scholars emphasize the close relationship between the circular principle of the Book of Changes and TCM thinking. The circular principle of the Book of Changes is a dynamic cycle; everything exists in periodic recurrence, and this principle has significant implications for TCM theory. First, the circular principle lays the foundation for the whole; the whole is a circular whole, and the holistic view is essentially a manifestation of a circle. The mutual interaction of Yin and Yang, the generation and overcoming of the five elements, are all circular phenomena; TCM’s time medicine is also a specific reflection of the rhythmic cycle of the Book of Changes. Qian and Kun are positioned; before Qian and Kun is Taiji, which is zero, the source of all things. Heaven and earth give birth to all things, not in a single completion, but in a continuous operation. All things are constantly being born, continuously rising and falling in the interaction of Yin and Yang.In symbolic numbers, zero is the application of the Taiji circular principle, in the continuous Yin-Yang interaction within the human body and the universe, rising and falling, using zero to connect the meridians and control the Bagua field. Therefore, “Curved shapes create all things without omission”; it will not miss anything because it is circular.The Qigong system’s methods are based on the “universal circular symmetry law”. The theorem is: self-relevance must return to infinity. The law is: the process of mutual correspondence among all things in the universe is a self-relevant, infinitely recursive process. This process forms countless mutually symmetrical circles. Each string of infinitely superimposed circles reenacts the process of cosmic evolution. Among these circles, corresponding parts are more similar than non-corresponding parts. This makes each circle a magnified or reduced image of all other circles. The proof is: self-relevance is multi-layered and holistic; infinity is the only whole, so the process of self-relevance is a process of returning to infinity in some way. From infinity to finitude, from finitude to infinity, the cycle continues endlessly, and the result of layer upon layer of return is the infinite superposition and symmetry of circles. This symmetrical circle set is evidently a microcosm of the cosmic process, and symmetrical circles must be similar; those that are similar are essentially mutually inclusive.In order for humanity to receive various information from the universe and achieve harmony between heaven and humanity, TCM uses circular medicine, Yi studies use circular explanations, Qigong uses circular practice, and symbolic number therapy uses circular healing.3. Further Discussion on One Key Opening One LockGeneral practices such as Qigong and body refinement are also methods. Since each person’s psychology is different, the methods also vary. The same method cannot apply to everyone.I deeply experienced this in 1998 when I self-treated shoulder periarthritis. Initially, my right shoulder was uncomfortable, and I selected the symbolic number formula:80·20; after a week with no effect, I changed the formula to8000; then changed to8000·70; and then changed to0004000, etc.; I self-treated for over a month with no effect.My right shoulder was painful, and I could not sleep at night. Without pre-heaven, there can be no change; without post-heaven, there is no foundation. This essentially refers to the practical Bagua diagram, which uses pre-heaven numbers and post-heaven methods, including “change” and “foundation” as a unified practical diagram, commonly used in predictive studies.First, I took the practical Bagua diagram, symbolic numbers, and positional correspondences to the body: Xun (5) corresponds to the left shoulder, Kun (8) corresponds to the right shoulder. Xun represents wood, which has the characteristics of growth and flexibility, thus I took050as the first element; the use of zero before and after the number is for the effects of left and right rotation, promoting growth and flexibility, and also represents the thinking of damaging the left and supplementing the right to achieve Yin-Yang balance.The second element takes Li fire to generate Kun earth; Kun earth has the characteristics of bearing and nurturing, being the mother of all things, but cold earth cannot generate; Kun earth prefers Li fire for application, as fire has the properties of heating, warming, and rising, thus the symbolic number formula is050·380.Using TCM’s hidden symbolism theory to validate this method is feasible; Xun represents wind, which has the characteristic of movement; in terms of disease mechanisms, anything active, changeable, wandering, or spasmodic belongs to “wind”; wind Qi corresponds to the liver and heart; Li trigram, representing fire, corresponds to the heart, which governs blood vessels, and the heart has the function of promoting blood circulation to nourish the entire body. The Li trigram also governs the San Jiao, which is the pathway for physiological functions, serving as the original energy pathway of the body, originating from the kidneys, stored below the navel, and distributed throughout the body via the San Jiao, promoting the activities of various Zang-Fu organs. The spleen, represented by Kun trigram, corresponds to number 8, which governs the muscles and limbs; the normal functional activities of the body are closely related to spleen Qi. When spleen Qi is strong, the clear Yang Qi flows throughout the body, and when nutrition is sufficient, the muscles are full, and the limbs are agile and strong.380; it also serves to warm and supplement the deficient Yang. Practical verification shows that this method is effective; after a month of self-treatment, I returned to normal work.4. The Effectiveness of Loudly Reciting Symbolic Number FormulasIn practice, while self-treating my shoulder, due to unbearable pain, my mood became anxious and unbalanced, making it impossible to calmly recite the symbolic number formula. I felt a sense of “desperation leads to calling out to heaven, extreme pain leads to calling out to mother.” I loudly shouted050·380; after a few loud shouts, I felt a certain energy, as if I had found confidence, and combined the timing of the Xun trigram with the timing of the Kun trigram to recite numbers for healing, and my shoulder began to improve. In a way that does not disturb others, using recitation or loud shouting methods is effective.
