Distinguishing Between Yin and Yang Patterns

In the year of the Yuan Dynasty, during the fourth month, a 19-year-old son of the magistrate of Jingjiang fell ill with a cold for nine days. The physician treated him as a Yin pattern and prescribed Fu Zi Li Zhong Wan (Aconite and Regulate the Middle Pill) several times, but his condition worsened. Another physician diagnosed it as a Yang pattern, leading to confusion and hesitation to take medicine. The magistrate’s son came to invite me to resolve the doubt. I declined due to potential bias, but he bowed and wept, saying, “If the great physician does not come, my son will surely die.” I had no choice but to go and see him. There were several people present, and I did not want to speak directly about his condition, but rather explained it in detail so that he could assess it himself.For a Yang pattern, the body must be very hot, with hands and feet not cold. When lying down, the person is calm; when standing, they have strength. They do not dislike the cold but rather dislike heat. They do not experience nausea or diarrhea, are thirsty and drink water, feel restless and cannot sleep, can eat and talk a lot, and their pulse is floating, large, and rapid.For a Yin pattern, the body is not hot, and the hands and feet are cold. They dislike the cold and curl up when lying down, facing the wall. They dislike hearing voices, may cover themselves with clothing, do not feel thirsty, do not want to eat, urinate freely, and have loose stools. Their pulse is deep, thin, and slightly slow.Upon examining his pulse, it was deep and rapid, reaching six or seven beats. His mother said he had been calling out all night and could not sleep at all. He also liked ice water. Upon hearing this, I recognized all the signs of a Yang pattern, and since he had not had a bowel movement for three days, it was necessary to purge him urgently. I then weighed out six qian of Da Huang (Rhubarb), two qian of Zhi Gan Cao (Honey-fried Licorice), and two qian of Mang Xiao (Glauber’s Salt), and decocted them in water. By evening, he had several bowel movements, expelling over twenty dry stools. That night, he sweated profusely. The next day, I went to see him again; his body was cool, and his pulse was calm. I recalled the discussions in the Suwen (Plain Questions) about heat, stating that treatment should open the organs. Therefore, Zhang Zhongjing’s Treatise on Cold Damage describes the six channels differently, with various transmissions. The Book of Living People also states that in treating cold damage, one must first clarify the meridians. If one does not understand the meridians, they will be lost in the dark. The ancients and moderns all agree on this. Those who do not study the meridians and do not inquire about the source of the illness, but rather blindly press and grasp, will misjudge the disease and not know where the evil qi resides, leading to severe consequences. Han Yuguong said, “The physician’s illness lies in little thought; the principle is to reach the words, encouraging people to learn and save lives with a heavy heart.”
